Information for an outpatient
For a new patient at our hospital.
Please enroll yourself at the first floor
of building No.1. Please fill in an application
form, and hand in the form, your health-insurance
certificate and the letter of introduction
from the introducing doctor, if you bring
it, to the reception desk No.3. Please wait
for a moment till a guidance of a person
in charge.After enrollment, please wait before
the counter of the consultation room until
you are called for a medical examination.
If you bring the data or the material from
the introducer, please hand in them to the
counter.
attention: A patient without a letter of introduction
has to pay the specific fee in addition to
the fee charged for a patient's first visit.
For a patient previously having a medical
examination at our hospital.
1.For a patient who consults a doctor of
a different department or who comes after
a long interval (more than 6 months).
Please hand in your consultation ticket of
our hospital and your health-insurance certificate
to the counter of the consultation room and
wait until you are called for a medical examination.
If you bring the data or the material from
the introducer, please hand in them to the
counter.
attention: A patient without a letter of introduction
has to pay the specific fee in addition to
the fee charged for a patient's first visit.
2.For a patient with an appointment with
your doctor.
Please hand in your consultation ticket of
our hospital, your appointment card and your
health-insurance certificate to the counter
of the consultation room by 10 minutes before
the appointment time.
If you will consult doctors of plural department,
please apply to the counter of department
with the earliest appointment time.
If you will consult after the special examinations,
please hand in your consultation ticket of
our hospital, your appointment card and your
health-insurance certificate to the reception
desk No.2 at the first floor of building
No.1 first.
If you want to change the appointment day
or time, please contact the counter of the
department from 10:00am. to 0:00pm. on a
consultation day not later than 3 days before
the appointment day.
3.For a patient without an appointment with
your doctor.
Please hand in your consultation ticket of
our hospital, your appointment card and your
health-insurance certificate to the counter
of the consultation room. When you have taken
a turn to the worse rapidly, please offer
that to the counter.
attention: The consultation of the patient
with appointment may be given priority.
4.For a patient who visit only for the special
examinations.
Your consultation ticket and the stub of
your reservation card for the examination
to the reception desk No.2 at the first floor
of building No.1 first. Please tell your
name to the examination stuff and wait until
your name is called for the examination.
When you have taken a turn to the worse while
waiting, please offer to the near window
without hesitation.
Information for admission
Procedure for admission
After finishing the procedure for admission,
we phone you to let you know when you can
be hospitalized. Please make preparation
for admission. Of course, you can be hospitalized
as soon as possible in case of emergency.
At the time of hospitalization, please show
us the following;
1) Health Insurance card or other card such
as service card for child (less than 6 year
old) [If you are injured with traffic accident
or while you work, we may not need the card.
It is because other sources may be necessary
at that time.]
2) Documents for renting bedding.

All visitors need to make signature in the
document for visitor on the counter in the
wards.

Only family member can visit Emergency Critical

toilet articles, cup, chopsticks , spoon,
tissue, towel, underwear, gown, slippers,
etc.
Restriction for a cellular phone
A cellular phone sometimes affects sophisticated
medical machinery, causing the severe problem
in treating patients. Therefore, we restrict
a cellular phone in Hamamatsu Medical Center.

1) On admission, we will give you orientation
by nurse.
2) While your hospitalization, please not
park your car in our parking lot.
3) If you need the document for insurance,
let us know on the counter in the ward.
4)If you change your address or health insurance
card, let us know on the counter #1 for admission
on the first floor.
